Transferring Records from Doctors of Northcote
This applies with you are:
- Requesting records held in relation to you (Ages 14 years and over)
- Requesting medical records for a child aged 13 years and under (parent or legal guardian)
- Requesting records for a deceased patient (authorised Executor)
- Guardian/Attorney requesting records on behalf of a patient
What we need to process your request:
- A completed online request form with signed consent and proof of identity (which will require your signature and consent, as well as proof of identification)
- Payment of an administrative fee (our team will email or post you an invoice)
- Please allow up to 5-weeks for processing
