Chronic disease is any health condition that has been, or is likely to be, present for more than six months. These may include:

To help you manage your chronic disease, our GPs can work with you identify all your health and welfare needs and the services required to address them. This is done through a Chronis Disease Management Plan, also known as a GP Management Plan (GPMP).
