Many people live with chronic disease. It’s not easy; it can be extremely stressful and can have some significant life changes. Working closely with your GP can assist the best health outcomes possible for you and your loved ones.

Chronic disease is any health condition that has been, or is likely to be, present for more than six months. These may include:

To help you manage your chronic disease, our GPs can work with you identify all your health and welfare needs and the services required to address them. This is done through a Chronis Disease Management Plan, also known as a GP Management Plan (GPMP).
